    For those readers who are curious as to what the medical literature in English has published about the use of the drug misoprostol (brand name: Cytotec) for cervical ripening or induction, I offer the following brief summary of the articles I was able to obtain. For your convenience, I have listed these articles in chronological order of their publication. This letter is the first mention in English of the use of misoprostol to induce labor. Findings: “Side effects were not observed, but polysystole (more than 5 contractions in 10 min.) was more frequent in the misoprostol group (17%) than in the oxytocin group (12%); however, no fetal distress was associated with this condition in either group.” However, this last statement appears in contradiction to another statement appearing in their letter. Labor is induced in more than 13 percent of deliveries in the United States. Oxytocin is the drug of choice for labor induction when the cervical examination shows that the cervix is favorable. The use of this agent requires experience and vigilant observation for uterine hyperstimulation, hypertonus or maternal fluid overload. In a patient whose cervix is unfavorable, the use of prostaglandin analogs for cervical ripening markedly enhances the success of inductions. Misoprostol, a prostaglandin E analog marketed as a gastrointestinal mucosal protective agent, is safe, efficacious and inexpensive for use in cervical ripening and labor induction. Further studies will better delineate its optimal use. Family physicians need to be familiar with the various methods of cervical ripening and labor induction. In an ideal world, all pregnancies would go to term, and labor would begin spontaneously.
