    Prednisone while breastfeeding

I have a severe case of plantar fasciitis, like I can barely walk. I normally wouldn't consider taking it but my pain is so bad. I was able to do a little reading and it seems that it's not uncommon for pregnant/nursing moms to take it. I haven't done a lot of research yet but just wondering if anyone has taken prednisone while breastfeeding. I still feel a little weird about it but it helps to see all the stories from other moms that didn't have any issues. Wonder if I could just take it before bed since baby doesn't nurse all night. From lactmed, the US National Library Medicine free database that this considered the gold standard on research of medicines and breastfeeding. Drug Levels and Effects: Summary of Use during Lactation: Amounts of prednisone in breastmilk are very low. No adverse effect have been reported in breastfed infants with maternal use of any corticosteroid during breastfeeding. With high maternal doses, the use of prednisolone instead of prednisone and avoiding breastfeeding for 4 hours after a dose theoretically should decrease the dose received by the infant. The infant's medication exposure can be limited by prescribing medications to the breast-feeding mother that are poorly absorbed orally, by avoiding breast-feeding during times of peak maternal serum drug concentration and by prescribing topical therapy when possible. D., Conemaugh Memorial Medical Center, Johnstown, Pennsylvania LUIS S. D., Conemaugh Memorial Medical Center, Johnstown, Pennsylvania DONNA J. The transfer of medications into breast milk depends on a concentration gradient that allows passive diffusion of nonionized, non-protein-bound drugs. A drug that is safe for use during pregnancy may not be safe for the nursing infant. Prescribing medications for a breast-feeding mother requires weighing the benefits of medication use for the mother against the risk of not breast-feeding the infant or the potential risk of exposing the infant to medications. Mothers of premature or otherwise compromised infants may require altered dosing to avoid drug accumulation and toxicity in these infants. The most accurate and up-to-date sources of information, including Internet resources and telephone consultations, should be used. Physicians receive little education about breast-feeding and even less training on the effects of maternal medications on the nursing infant.1 Yet, concern about potential harm to the nursing infant from maternal medications is often cited as a reason to advise discontinuation of breastfeeding.

steroids are essential to reverse the eosinophilic airway inflammation which causes symptomatic exacerbations of asthma. Much of the current variation in clinical practice is not justified by data from clinical trials. Oral prednisolone is as effective as intravenous therapy and very high doses of corticosteroid are no better than modest doses (30-50 mg prednisolone). Corticosteroids should be given twice a day for optimum effect. Therapy does not need to be tapered, but can be ceased abruptly after 10 days in most patients who are also taking high -dose inhaled corticosteroids. There is an increasing role for inhaled corticosteroids in the management of mild exacerbations of asthma. The dose, route and duration of therapy need to be defined for each patient and written down as part of an action plan to enable early intervention in future exacerbations.
